NY Department of Health Looking for Input Regarding Advance Care Planning

The New York State Department of Health (NYS DOH) is gathering information from stakeholders who use advance care planning documents and/or have advance care planning and end-of-life care conversations with patients and their families.

Physicians who use the New York State Department of Health MOLST Form or eMOLST are asked to provide input about use of MOLST and other advance care planning documents. DOH intends to address practice patterns and appropriate changes to improve the process and the forms.

Anyone who wishes to provide input to the Department of Health regarding MOLST, Nonhospital Do Not Resuscitate, and Health Care Proxy forms can do so here, or can email ALTCteam@health.ny.gov. The deadline for comments and feedback is November 16.

The Chapter has already provided comments and feedback, but the strength of improving the MOLST process is dependent on as much feedback as possible from physicians who engage in Advance Care planning discussions.
