This survey of Non NYS PCMH Recognized providers is a collaboration between the New York State Department of Health and the New York Chapter of the American College of Physicians. This survey is designed to collect information about your decision to not participate in the NYS PCMH Recognition program, and to help identify further educational strategies that will support internal medicine practices not participating in NYS PCMH. The survey will take no longer than 5 minutes to complete.
