The Near Miss Project is being sponsored by the New York Chapter American College of Physicians, Association Program Directors Internal Medicine, New York State Special Interest Group (APDIM NYS-SIG) and supported by the New York State Department of Health. (not applicable to Medical Students)
Purpose
The purpose of the project is to collect data relevant to “near misses” in patient care in a safe, anonymous and risk-free environment. Near Misses are those events that might have resulted in harm to a patient but were discovered and corrected before they reached the patient.
By reporting a near miss event, reidents will receive a printable certificate upon completion of the survey. This certificate will contain no specific information about the entry; it can be signed and given to their Program Director as evidence of your competency in Practice Based Learning and Improvement, Professionalism and System Based Practice.
We believe that hospital morbidity and mortality and other reportable occurrences represent only a fraction of the medical errors that are present in health care. Studying near miss occurrences can tell us much more about our patient safety vulnerabilities as well as the strength and integrity of the barriers to errors that protect our patients. In fact, the Near Miss Survey is also the only tool available that is specifically designed to examine the barriers or safeguards that protect our patients. Through a web based tool, located @ www.nearmiss.org, hosted by NYACP, we hope to prove that voluntary, anonymous reporting is an effective way of identifying vulnerabilities in our health systems.
Our desire is for all 4,628 Internal Medicine Interns and Residents in New York State to voluntarily report at least one near miss event.
