Instructions:
There is a $30 fee per abstract submission. Use this form ONLY if you are paying by CREDIT CARD.

How To Submit Your Abstract:

After your credit card payment is successfully processed, a link to submit your abstract is provided on the "Thank you" page that IMMEDIATELY follows. You must submit your abstract one at time of payment. If you close your browser and return later, the link may be lost.

Paying By Check:
You can send a check or money order, made payable to the NY Chapter ACP and mail it to: PO Box 38237, Albany, NY 12203. Once your payment is received we will then send you a link to submit your abstract. All payments made by check or money order must be received by Monday, January 18, 2021 and you will have until Friday, January 22, 2021 to submit your abstract.
